Recovery is an ongoing process that requires continued commitment, outpatient therapy, and support after discharge from our treatment facility. At The Healing Connection, patient discharge is carefully planned and coordinated among patients, families, clinicians, primary care providers, and insurance providers. Our attention to individualized discharge planning ensures a smooth transition into continued coordinated care.
Patients and families work with our multidisciplinary team of mental health providers to design and implement a comprehensive plan that promotes relapse prevention and a successful transition back to day-to-day living. Activities will help patients address eating habits, stabilize their weight, and tackle psychological and behavioral symptoms associated with eating disorders.
Our staff is well-equipped to recommend specialists across the region, including mental health therapists, psychiatrists, and nutritional counselors/dieticians as well as local mentoring and support groups.
